Covid clinic at UAHT

HOPE – UAHT is hosting a COVID Vaccination Clinic on Wednesday, April 7, from 10a.m. to 4p.m. in Hempstead Hall on the Hope Campus.  The vaccine administered will be Moderna which can be given to individuals 18 years of age and older.  The clinic for the second dose will be Thursday, May 6.  There is no out-of-pocket cost for the vaccine.  If you have health insurance, it will be billed, but you are not responsible for any co-pay or deductible.

    *  Allows 8 people to sign up in each 15 minute time block
COVID-19 protocols will apply, including:
-Wear a face mask.
-Observe social distancing.
-Be prepared to remain on site for 20 minutes after vaccination.
-Wear loose fitting shirt or blouse for easy access to upper arm.
The following documents will be available to be downloaded and printed upon signing up – to allow the vaccine clinic process to flow smoothly, please arrive with a completed consent form (front & back) along with your insurance card (or if Medicare Part B, the red/white/blue card):
  *  Immunization Consent Form = each person should fill out and complete the form and answer the COVID vaccine screening questions – front and back (have insurance cards or a copy of them handy; if uninsured, indicate as such to the pharmacist)
  *  Moderna EUA Recipient Fact Sheet = Moderna’s COVID-19 Vaccine fact sheet for vaccine recipients
  *  V-Safe Information Sheet = V-safe is a vaccine safety health check-in tool from the CDC
